FASD Training Evaluation Survey
Thank you for attending the Learning Disabilities Association of New York State’s Fetal Alcohol Spectrum Disorder Professional Development training.We are conducting an evaluation survey to assist our efforts to quantify how participants who attended our training changed or created practices, teaching methods, screening policies, etc. that will serve as indicators that the following long term goals will occur as a result of attending our training session on FASD:
- An increase in the number of referrals to service providers who can provide support to families and individuals (i.e.: substance abuse, disability services, special education, etc.).
- A reduction in the number of women using alcohol during pregnancy as a result of professionals providing accurate information and guidance.
- A reduction in the number of individuals with FASD who become dependent on alcohol.
- A reduction in the number of females with FASD who use alcohol during pregnancy.
There are no risk associated with completing the evaluation survey. We believe that the information we receive from this questionnaire will support our mission to increase public and professional awareness of FASD to further its prevention.
If you have any comments or concerns about the questions reflected in the survey, please include them in the section devoted to comments or suggestions on page 4 of the evaluation survey.
